Myocardial Infarction and Ischemia, I: ST Segment Elevation and Q Wave Syndromes

چکیده

MYOCARDIAL ISCHEMIA Myocardial cells require oxygen and other nutrients to function. Oxygenated blood is supplied by the coronary arteries. If severe narrowing or complete blockage of a coronary artery causes the blood flow to become inadequate, ischemia of the heart muscle develops. Ischemia means literally “to hold back blood.” Myocardial ischemia may occur transiently. For example, patients who experience angina pectoris with exercise are having transient myocardial ischemia. If the ischemia is more severe, necrosis of a portion of heart muscle may occur. Myocardial infarction (MI) refers to myocardial necrosis (or, in nontechnical parlance—a “heart attack”). This discussion focuses primarily on ischemia and infarction of the left ventricle, the predominant pumping chamber of the heart. The important clinical topic of right ventricular infarction is also discussed briefly.

Myocardial Infarction and Ischemia, II: Non–ST Segment Elevation and Non–Q Wave Syndromes

82 Myocardial infarction (MI) may be associated with abnormal Q waves associated with the typical progression of ST segment elevation myocardial infarction (STEMI) and T wave inversions described in Chapter 8. In many cases, however, myocardial ischemia (with or without actual infarction) may be limited primarily to the subendocardium (inner layer) of the ventricle, often associated with ST seg...

Utility of the prehospital electrocardiogram in diagnosing acute coronary syndromes: the Myocardial Infarction Triage and Intervention (MITI) Project.

OBJECTIVES We sought to determine whether the prehospital electrocardiogram (ECG) improves the diagnosis of an acute coronary syndrome. BACKGROUND The ECG is the most widely used screening test for evaluating patients with chest pain. METHODS Prehospital and in-hospital ECGs were obtained in 3,027 consecutive patients with symptoms of suspected acute myocardial infarction, 362 of whom were ...

Non St Elevation Acute Coronary Syndrome: Contemporary Management Strategies

The term, acute coronary syndrome (ACS), describes a spectrum of clinical conditions ranging from ST-segment elevation myocardial infarction (STEMI) to non–ST-segment elevation MI (N–STEMI) and unstable angina (UA) (Fig. 119.1) (1). These manifestations of acute myocardial ischemia may revert to the presymptomatic state or evolve into a non-Q wave (also termed nontransmural) MI or to Q wave (or...

ST segment shifts are poor predictors of subsequent Q wave evolution in acute myocardial infarction. A natural history study of early non-Q wave infarction.

Acute ST segment elevation is regarded generally as the sine qua non of evolving Q wave myocardial infarction (MI) because such electrocardiographic (ECG) injury is believed to be a marker of transmural ischemia and a forerunner of transmural necrosis. Alternatively, ST segment depression with or without T wave inversion is viewed as the dominant ECG feature of non-Q wave MI. However, this hypo...
